Vaccination Requirements
This is a reminder that North Carolina law requires all children in the state to receive certain immunizations. Records are checked when children enter school, and there are certain grade levels that require specific vaccines.

For students entering the seventh grade, state law requires all adolescents to be up to date on all vaccinations and to receive one dose of Tdap vaccine and one dose of meningoccal conjugate vaccine.

For students entering Kindergarten, state law requires every child have complete vaccinations. A copy of the complete vaccination record must be presented to school on or before the first day of school.

In addition, state law requires that every child entering kindergarten receive a health assessment. The assessment must occur within 12 months prior to entering school. The medical provider, parent or guardian must provide a completed health assessment transmittal form to the principal of the school on or before the first day of school.
